At Thrive2Ability Care, our referral process is designed to be simple, clear, and supportive for both clients and their families. It usually begins when a participant, family member, or support coordinator contacts us to discuss the individual’s needs and goals. Our team then conducts an initial assessment to understand the type of support required and how we can best assist.
Once we have a clear understanding, we guide the client through the necessary documentation and approvals, including any relevant NDIS plan details if applicable. After the referral is confirmed, we create a personalised support plan tailored to the participant’s goals, abilities, and preferences, and begin providing services in a coordinated and consistent manner.
Throughout the process, we maintain open communication, answer any questions, and ensure both clients and their families feel informed, confident, and supported every step of the way.
