At Thrive2Ability Care, our referral process is designed to be simple, clear, and supportive for both clients and their families. Once we have a clear understanding, we guide the client through the necessary documentation and approvals, including any relevant NDIS plan details if applicable. After the referral is confirmed, we create a personalised support plan tailored to the participant’s goals, abilities, and preferences, and begin providing services in a coordinated and consistent manner.
